Tax Forms

Massachusetts and Federal tax forms are available in the library. We receive the MA Department of Revenue’s Form 1 for both Residents and Non-Residents, and Federal forms as part of the IRS Tax Form Outlet Program. As such, we are entirely reliant on them for the availability of tax forms.

If we do not have copies of the form you need, our Reference Staff can print them for you. We will print one copy of each tax form at no charge, and the cost for additional copies, or for instructions, is $0.15 per page. Forms and instructions are available online:

Free Tax Assistance

Each year, an AARP Tax-Aide volunteer tax consultant will be available at the main library to offer free tax assistance. Space is limited! Appointment scheduling will begin on January 15th, 2026. 

AARP Tax-Aide volunteer tax consultants will be available at the main library on Monday and Wednesday evenings starting in February to offer free tax assistance. 

Tax-Aide services are open to all taxpayers except in these instances:

a) The tax return has issues that are out of scope, as defined by Tax-Aide.  Examples of out of scope issues include Crypto currency transactions and rental income.

b) The scale of the return, or the number of tax documents, makes it impossible to complete the return within a reasonable amount of time.

Determinations are the responsibility of the Tax Aide local or shift coordinator.

All taxpayers should complete an intake package before their appointment. Forms are available at the Library Reference Desk. We can also email the forms to you. Please call (978) 364-0199 and leave a message if there are any questions. We will return your call as soon as we can.

Please bring the following with you when you come:

  • A photo ID for you (and your spouse if filing jointly).
  • Social Security card or an SSA-1099 form for anyone on your return including yourself, spouse, and/or dependents
  • Completed Intake form
  • Copy of last year’s income tax return(s)
  • W-2 forms from each employer
  • All 1099 forms such as:
    • 1099-INT, 1099-DIV, 1099-NEC., Etc.
    • SSA-1099 form if you were paid Social Security benefits
    • 1099-R form from the payer of your pension or annuity
    • 1099-HC for proof of Healthcare
  • Any forms indicating federal income or state income tax withheld
  • Bank Account and Routing numbers to be used for  direct deposit of refund or tax payment (these numbers can be found on a checking account check)
  • Homeowners that may qualify for the MA Senior Circuit Breaker credit should bring record of property tax and Water and Sewer payments.
